A record of the immunisations you’ve received, including COVID-19 vaccinations, is available online at My Health Record.

Read more about My Health Record(external link) and find out how to set up and use My Health Record in the more information section below.

Your immunisation record will include:

  • the name of the vaccine
  • the batch number of the vaccine
  • the dose number, for example dose 5 means you've had five COVID-19 vaccinations
  • the country where and the date when you were given the vaccine
  • any overseas vaccinations that have been added to your health record based on records provided by the countries you received the vaccination in.

You can also call the Vaccination Healthline free on 0800 28 29 26 to get your immunisation information.

Official immunisation record

An official record of your vaccination information, which provides the batch number and date the vaccine was given, can be emailed to you. Request this online at My Health Record or by calling the Vaccination Healthline.

The information on your immunisation record comes from the Aotearoa Immunisation Register (AIR).  All COVID-19 immunisations will appear on your record.

  • They were paid for privately and may not appear unless you asked your healthcare provider to add them to the national register.

You can correct or update any missing information on your immunisation record by contacting My Health Record. You can:
