Te Ara Pae Ora is a health and hauora app to support whānau to connect to local health, social and community services that can support you in the areas that you need. The app gives users control about the information they want to share with other support people or service providers.
The app has provision to create a hauora wellness plan with your care team. The plan helps your healthcare providers to understand your hauora (health and wellbeing) and support you to be as well as you can be.
Currently the app is only available for people living in the Kāpiti, Porirua, Wellington and Wairarapa regions.
