ProCare

In 2019 ProCare launched their Population Health Strategy which outlines how we are going to make the biggest difference to the health of our enrolled population over the next five years.

ProCare has teamed up with Healthify to provide a range of resources, information and support services centred around their strategy’s 5 clinical health goals – children 0–4 years, youth 15–24 years, older people’s health, wellbeing and preventive care and long-term conditions.
